It looks like a phalaenopsis orchid!
Is it still in the medium in which you bought it? I have found a lot of orchids in shops are packed around a plug that causes root problems, or that the medium is not very suitable. Also, how does the flower spike look? Did you trim it back to a node with a sheathed spike, or have you let it be? Is it turning brown at all?
I would definitely re-pot your orchid in orchid bark in an orchid pot, the roots need ventilation. I've found the best results repotting and removing that spongy plug careful from the base of the plant and roots. I got my orchid pots here

Thanks everyone! The plant is in the soil it came in, and the teacup had come from another plant, but it was the same size that the orchid's original pot was. I think it was overwatered by the time I got it (it came from Home Depot). I don't notice any browning of the leaves at this point, but a couple new leaves have grown in the last year and I do have 3 small new shoots coming from it (you can see from the pictures, I think). I trimmed the flower shoots all the way down to the leaves because the whole thing was dead and brown and dry, not sure what a sheathed spike looks like (clearly I'm new to orchids), and I haven't touched them since I did that last year. I will look into other soil and a different pot!
