Why are some of the leaves not opening up? Then they die.

Calatheas need lots of humidity... a lot. I don't know if you have it around a humidifier or not but I find that they are essential for this type of plant. Also when it's actively growing fertilizer is your friend given that you're using it as instructed but I would halve the amount per dose each time.
