My first guess would be too much water / the soil stays too moist for too long. That is almost the only thing you can do wrong with these.

Do a repot and check the roots for root-rot. Cut of the bad parts of leaves and roots. Give it well draining soil in a pot with drainage so it won't stand in water. And don't water before it is completly dry.
Looks like it's rotting from overwatering and/or the soil staying damp for too long. What type of soil do you have it in? It looks to be far too high in organic matter and/or peat moss, so it's retaining too much water. Snake plants are succulents, so they need a gritty, well-draining soil that dries out quickly. Also, they should only be watered when the soil has completely dried out, which depending on a variety of factors in your environment could be anywhere from once every 2-3 weeks to once every 4-6 weeks. They're drought tolerant plants, so it's always better to underwater than overwater (underwatered plants are an easy fix, overwatered...not so much).

I'd recommend using soil specifically made for cacti/succulents with extra perlite mixed in (3 parts succulent soil, 1 part perlite). Your pot setup is also contributing to the soil not drying out fast enough. Plastic is non-porous, so it holds onto moisture. Then when you place the pot inside another pot, the restricted airflow between the two pots keeps the soil damp for even longer. All my snake plants are in terracotta pots, which are recommended for succulents because they're porous and allow the soil to dry out faster. When you remove it from the pot, inspect the roots for any signs of rot (mushy, darker in color, fall apart easily when you handle them). Any rotted roots will need to be removed with scissors or pruners sterilized with 70% or higher isopropyl alcohol before repotting. The leaf in your photo will also need to be removed by cutting as close to the base as possible.
