The soil looks really wet and I see on you plant card that the pot has no drainage. If this is the case it’s being overwatered. Repot it in a pot that has good drainage and put it in fresh soil. While you’re at it, check the roots to make sure there is no rot going on. If there is any black, brown, or mushy roots cut them away with sterilized shears and treat it with hydrogen peroxide.
