I’d need better photos to make a more educated guess, but from my experience, it might be “drowning” if that makes sense? Orchids are swamp plants by nature (hence they do well in loose soil/mulch). While they can be grown in nothing but water, they also need good drainage when in a substrate. Changing to a pot with drainage may help.
