This can happen when water sits on the leaves too long. Indoor plants don't have the benefit of great air flow like outdoor plants, so moisture sits on leaves and stems longer. Try not to get them wet when you water, and if you do then dry them off with a soft, clean towel. With such a small plant, it helps to use a squeeze bottle with a narrow spout for watering. If gives you much more control over where the water goes, so that only the soil gets wet and not the leaves or stems.
What does the soil feel like right before you water? Jade are extremely drought tolerant (they can go a very long time without water), so you want to make sure the soil completely dries out between waterings. If the soil stays too damp or doesn't have a chance to thoroughly dry out, it can cause it to rot. When in doubt, it's always better to err on the side of underwatering rather than overwatering.
One last thing, your plant is growing at a slant and stretching because it's not getting enough light, and the light it does get is only coming from one side. These guys need a ton of direct sun to thrive, at least 4-hours of direct sun from either a west or south-facing window.
