I have this flower in my flower bed what am I doing wrong

Mine looks like that this year too. In my case bits of it died off during freezing weather and then some rotted during relentless rainy weather. And then the living bits grew amongst the dead leaves. Has yours been in the flower bed all winter or has it just been planted? 🌿💚
