Help! I think my snake plant is dying. Can I save him ?

@varu Welcome to the Greg community. I can’t tell, first a few questions: does the pot the plant is in have drainage holes? The soil in the picture looks very wet? Snake plants are very drought tolerant and need very little water. I not sure if you can still save the plant, but it’s certainly worth a try. Remove it from the pot, and carefully check out the roots if they are nice and firm leave them be. But If they are brown and squishy or smell cut them off. I would also remove any leaves that look brown and yellow as those will not recover. And you want to re-pot the plant in fresh soil that is well draining.
