What has happened to my beans? too much water? Sun? I just added more soil and nutrients

That pot is way too big for such a tiny plant, so after watering the roots absorb what they need and then are left sitting in a bunch of excess damp soil. This can cause them to rot. Adding more soil will only worsen the situation. Seedlings are very fragile and can very easily be harmed by overwatering and over fertilizing. A young plant of this size should be in a much smaller "starter" pot until it's big enough to be transplanted to a larger one.
