Erhm, sorry, it seems it's a bit late for that. I'm case the description is correct and that once was a phalaenopsis orchid, it is most certainly dead. Probably from crown or stem rot. Phals grow upwards along their stem, producing new leaves only from the top (the crown). Once that is destroyed they can no longer grow new leaves. If they are otherwise healthy, they can still survive, even bloom, for a while - sometimes long enough to grow an offshoot on the base or flower spike to survive that way one the older leaves have died off one by one.
In your case there are no more leaves, the stem is dead. Sadly no amount of roots will help: the only way to grow something new would be a section of healthy stem .
The roots also seem dead. Next time be careful when watering not to splash any on the leaves and especially the stem to avoid root. Only water the roots. Don't keep it continually most, water when the roots are drying out (turn from green and plum to a matte silver gray).
