It looks like this was grafted onto a different plant so really it's up to you- you could probably let it grow for awhile then remove it when it's a decent size and plant it by itself- or let it grow forever and have a really unique plant! You could remove it but I would be careful with that just because an opening in the stem is always risky... good luck!

It's certainly not going to hurt the plant to leave the new leaf or arm in place. Leaves help the plant to photosynthesize, and this is how the plant nourishes itself. I wouldn't cut it, as it is helpful to the plant.
