Why is my plant falling in on itself

Well, that's not too much info lol. But, going by your plant card, it needs as much sun as you can provide, most southern window you have. It says regular soil and cacti need gritty/chunky fast draining succulent/cactus soil in preferably a terracotta pot. And lastly, from "falling in on itself" really sounds like you over-watered it and it might be rotten. 😒 cacti and succulents only need water when the soil is ABSOLUTELY dry. You could even wait a little longer if need be, they don't need much attention, just a lot of light. I recommend taking it out of the dirt and checking out the roots. They should look white/off white with not much smell. If they're brown/black and soft and stinky it's rotten. Would you be able to add a current picture? Maybe a little more info like how often you water or anything else? I hope it works out for you!
I agree with @FairyFly that "falling on itself" most likely means it's been overwatered and got mushy. I also second the suggestion of moving it to a terracotta pot. Plastic is non-porous so it holds onto moisture a lot longer. Terracotta is porous, which improves airflow and helps soil dry out faster between waterings (they're recommended for all cacti and succulents). Find a terracotta pot no more than 2 inches bigger than the root ball (a 4-in terracotta pot should be perfect). In the meantime, I'd get it out of that plastic pot asap and remove as much wet soil from the roots as possible. When you a have a new pot ready to go, repot it in fresh soil (the kind made specifically for cacti is great, but if you're using regular soil make sure to add a lot of perlite or pumiceβ€”2 parts soil, 1 part perlite or pumice). Once repotted, put it in the brightest spot in your house (a south or west facing window if possible) and don't water again for at least a few weeks so it has a chance to dry out.
