Often, it's caused by the soil staying wet from overwatering and/or lack of drainage.

I agree. You've been over watering on the regular. Only water when soil in dry at least one inch from surface.. The best method is the finger test. Push your finger one inch or more into the soil to check. And you probably want to replace that soil. Good luck 😃
