Is my plant dying?! How can I revive it
Over watering. Or Slow draining soil . Is your pot has enough holes ?

I noticed your plant card states no drainage. I suspect that the roots are just over watered. Re-pot your sage into a pot with drainage, let it dry out before watering again and lots of sunshine. Sage is a perennial shrub that likes sunshine and air movement. They do well in pots outside, depending on your area.
