It could be due to low humidity or under-watering. I have mine planted in a chunky soil mix and only water it once the soil completely dries out. Mine is also at about 55% humidity. You may want to do a repot and inspect the roots as that soil looks really dense. (Adding perlite and orchid bark can help with this). I killed my first one but my second one is thriving!
