is this root rot?

Why am I getting brown patches at top of cactus? They are not soft and have some flaky “skin”. Don’t think it’s root rot, could be fungus?

I picked this up a week ago. It fills a 6 inch pot. Should I keep it indoors? Repot?

How do I help this guy get back to healthy?

What do you think, sunburn or root rot?

Not sure if this is a pest or if it got burned being in an east-facing window this summer. I did a Neem oil spray a few days ago to be safe.
